Primary Information
We recommend that you put your full First and Last name on your ID. We suggest that you use your given name rather than nicknames or preferred
names. Why? It is easier for emergency personnel to locate vital medical history if they have your given name. You may, however, want to add
your middle name, initial, nickname, or preferred name to your ID in addition to your given name (see below).
Below are a few examples of how to enter your name:

We recommend that you put your City, State, and Country on your ID. Zip Codes are not necessary. Why? Zip Codes are typically used for "mailing"
purposes only. First responders and medical personnel will not need to know how to mail you a letter. Instead, we suggest that you use the extra
space to list your Country. This could be a tremendous asset if you are ever involved in an accident while visiting another country.

Besides your name, phone numbers are the most important items on your ID! This is how first responders and medical personnel will contact
your family and friends. In the event of an accident, not only will you be able to have your loved ones notified quickly, but they will be able
to convey vital information to emergency workers. We recommend that you put at least 2 phone numbers on your ID. Be sure to list numbers
of people that are most likely to answer the phone when you may need them to. Many people will choose to list work numbers and neighbors also.
If you live alone, do NOT list your own phone number. Likewise, do NOT list your own mobile number. These numbers will not be an asset in the
event of an emergency.
NOTE: Phone numbers are easiest to read when they are entered with dashes, like this:
"859-341-1102"
Below are a few examples of various ways to enter phone numbers:

Secondary Information
If you have allergies to certain medications or food, we highly recommend that you indicate this on your ID. (NOTE: you do NOT need to list
any "seasonal" allergies).
There are many medically accepted abbreviations for allergies. Click Here to see a list of common allergies and their medical abbreviations.
IMPORTANT: You MUST include the word "NO" or "ALLERGY" when listing an allergy. Simply listing the word "PENICILLIN" may confuse medical
personnel - they would not know whether you are allergic to penicillin or if you are taking penicillin.

MEDICAL HISTORY) will help medical personnel and first responders know how to treat you.
IMPORTANT: Do NOT list this information if it does not apply to the person wearing the ID. For example, do not list NKA if a person is
allergic to penicillin.
NOTE: NKA is the medically accepted abbreviation for "NO KNOWN ALLERGIES." NKDA is the medically accepted abbreviation for "NO KNOWN
DRUG ALLERGIES."
NOTE: Always use the longest abbreviation that will fit on your ID. For example, if you have room on your ID to list your blood type
as "A POS BLOOD," do not list it as "A+."

If you have some extra space on your ID, we encourage to have some fun with the extra space. Be creative, go wild. Many people will use extra
space for inspirational quotes and slogans - something to keep them motivated (like "NEVER GIVE UP"). Others will list something humorous (like
"IF FOUND, TREAT WITH BEER"). Others choose list a monumental achievement (like "CHICAGO MARATHON 3:22").

Unimportant Information
If you need room to list other important information on your Road ID, you can easily omit your Address - it's just not that important. Why?
In the event of an accident, your Name and Emergency Phone Numbers are going to supply the first responders with the majority of the information
they need. If, for some reason, the authorities need to locate your home address, they will be able to easily find it with your Name, City and
State - all of which should be on your ID. If, however, you want to list your street address, please feel free to do so.

There are many words that are not important to list on your Road ID.
Below are a few examples and the reason why they are not important:
EMERGENCY or EMERG
It is not necessary to identify a Phone Number as the "EMERGENCY" number. First Responders are going to dial the phone numbers on your ID whether
they are labeled "EMERGENCY" or not. Indicating this is often a waste of valuable space on your Road ID.
DRIVERS LICENSE NUMBER
Contrary to popular belief, your drivers license number does not provide much information about you. In fact, if you have an accident after the
standard business hours of the local court house, it may not be possible to acquire any identifying or contact info from your DL number.
